Dead. Or not.

TO: All
FROM: Anon Doc

Hey, I just noticed in the system that Patient XYZ is listed as deceased, but there's no note.  Does anyone know what happened?  I saw him two weeks ago and he looked good.  Thanks.

TO: Anon Doc
FROM: Attending

Oh, wow, I saw him that visit too.  He did look good.  I wonder what happened.  Do you mind calling his family?

FROM: Anon Doc

Okay, I'll give them a call.  But it's weird there's no note, right?

Maybe the family called but didn't give any information.  Let me know.

I just called.  He answered the phone.  Not deceased.  I asked how he was doing.  He said he was fine.  Said I was just calling to confirm his next appointment.  Do you know how to change his status in the system from deceased to alive?  I'm not sure how to do it.

Not sure, sorry.  Glad he's alive.  Maybe ask the helpdesk.  Thanks.

    I overhead a conversation in the hospital about my father. Rumor has it that your records show that he is deceased. He is not deceased. He is very much alive. This conversation greatly concerned me because the difference between alive and deceased is more important that the difference between brown eyes and blue eyes or even male versus female.

    However, my father values the care you give very much. He says that the doctors all know him and remember him from previous visits. He says that Anon Doc even called to confirm his next appointment, and he feels very cherished.

    My husband and I are concerned that your records cannot keep straight whether a patient is dead or alive. We're worried that that reflects a deeper problem with your care. However, my father says that the doctors know him and that they hate the computer system as well.

    Patient XYZ will continue to be your patient, but, please be aware, your system is unnerving and the daughter and son-in-law are watching. One more mistake and we may have to rescue him!!

  3. My medical record at one physician's office says both of my brothers are dead. Not true. Another file at a different physician's office says my dad died from cancer. Also not true. I have no idea why my records say these things.

    As far as I know, my physicians all correctly think I am still alive, which is good, I guess.
